This initiative aims to provide blood glucose screenings in the community, distribute educational handouts/resources, as well as educate people about the importance of their blood glucose values.  This program is designed to raise awareness about diabetes and counsel patients with diabetes to optimize and improve their healthcare.

 

SNPhA actively educates patients on lifestyle changes, medication therapy, self-care, and appropriate monitoring for hypertension and stroke risk factors. We serve the community with outreach events and free blood pressure screenings to promote awareness. SNPhA has partnered with the American Stroke Association (ASA) on the Power to End Stroke/Smoking Cessation initiative.
