What is CAARE?

Healing with CAARE, Inc. (CAARE) is a non-profit organization that serves the uninsured, underinsured, and low-income populations in Durham and Research Triangle Park, North Carolina. It provides a holistic approach to improve the health and lifestyle habits of its patients and its variety of programs aim to treat both the medical and social/human determinants of disease. UNC SNPhA Pharmacy Clinic

We are so excited to be partnering with CAARE in our development of the UNC SNPhA Pharmacy Clinic. This clinic will be part of the Health and Wellness arm of CAARE. It aims to establish a pharmacy presence at CAARE in order to further the organization’s ability to provide holistic health services to its patients. The goal of the UNC SNPhA Pharmacy Clinic is to provide a means of addressing patient concerns related to medication use and safety and to help manage chronic disease states. Some of the FREE services which will be provided include:

  • Longitudinal assessment of vital signs (blood pressure, pulse, weight, height)
  • Blood glucose screenings
  • Diabetic foot care
  • Laboratory testing referrals
  • Comprehensive Medication Reviews (CMR)
  • Pill box filling
  • Patient counseling and education
  • Healthy cooking demonstrations
  • Food bank giveaways
  • Need-based provision of pill boxes, BP cuffs, glucometers, testing supplies and public transportation assistance.

Establish a pharmacy presence at CAARE in order to:

  • Provide experiential education for student pharmacists
  • Promote SNPhA’s mission of serving the underserved
  • Demonstrate the essential role of pharmacists in chronic disease management
